- BSN Employer Survey (pdf)
- This survey evaluates employers' satisfaction with and perceptions of graduates of Western's BSN program. This survey can be modified to evaluate employers' experiences with the graduates of any program.
- Entering Student Survey (pdf)
- This survey evaluates the decision-making process for first-time freshmen, as well as assessing the impact of various advertising and recruitment methods used by the university.
- Faculty Technology Survey
- This survey evaluates the use of technology in the classroom by faculty. It assesses the need for additional technology training, as well as evaluating faculty perceptions on the efficacy of these teaching tools.
- Graduating Senior Survey
- This GA survey evaluates graduating seniors's feelings on various University services, as well as their overall perception of their undergraduate career.
- Sophomore Survey
- This GA survey evaluates sophomore students feelings on various University services, as well as their overall perception of their undergraduate career at the midpoint.
- Summer Orientation Feedback Parents (pdf)
- This survey provides the Orientation Programs office with indicators as to parent satisfaction of the university’s orientation programs and services. It provides the Orientation Programs office immediate feedback judging our performance for use in implementing new processes and procedures as well as setting priorities in our action planning for program enhancement.

- This survey is similar to the Parent Orientation Feedback survey above, but targeted towards the students attending orientation.
- Transfer Student Survey (pdf)
- This survey is similar to the Entering Student survey above, but targeted towards incoming transfer students. It evaluates the decision-making process, assesses the impact of various advertising and recruitment methods used by the university, and gathers data on feeder schools and academic achievements prior to transfer.

- This survey evaluates the backgrounds, attitudes, and motivations of incoming first-year freshmen students.
